Advance Directives for Wisconsin Residents

It's never too late to plan ahead. Get the tools you need to assess your goals, values and beliefs about end-of-life care. We will offer tips on how to initiate a conversation with your loved ones and how to pick a surrogate decision maker. Participants will also be given an opportunity to schedule a free, private appointment to complete their advance directive with a certified Advance Care Planning facilitator.
